The accident led to high thyroid exposures, primarily due to 1311 in the cloud of radionuclides from the reactor. The relatively short half-life of 131I (8.02 days) means that the dose was received within several weeks of the accident. The Hanford Thyroid Disease Study

From 1944 to 1957, large amounts of radioactive substances were released into the atmosphere at Hanford, Washington. The Hanford Nuclear site occupies some 560 square miles in southeastern Washington State, and was the world's first large-scale plant for the production of plutonium for nuclear weapons. Over the course of years, large amounts of radioactive nuclides were released into both the Columbia River and the atmosphere. Iodine-131 was carried by winds to surrounding areas and deposited on vegetation, where it was taken up by grazing cows and goats. Hyperthyroidism is a condition in which there is sustained overproduction of thyroid hormone by the thyroid gland, resulting in elevated levels of the circulating thyroid hormones (i.e. elevated T3 and T4 see Box 2). Primary hypothyroidism is defined as elevated thyroid stimulating hormone (TSH) and low serum thyroid hormone concentrations, including thyroxine (T4) and triiodothyronine (T3 see Box 1). Hypoparathyroidism is a disorder in which there is deficient parathyroid hormone secretion (see Box 4). The condition can occur at any age and appears equally prevalent in males and females. Parathyroid hormone is responsible for maintaining calcium homeostasis in the body as well as regulating other ions, such as phosphate. The most common cause of hypoparathyroidism is surgical removal or damage to the parathyroid glands. The potential scope of cancer prevention is limited by the proportion of human cancers in which extrinsic factors are responsible. These factors include all environmental carcinogens, whether identified or not, as well as modifying factors that favor neoplasia of apparently intrinis origin (e.g., hormonal imbalance, dietary deficiencies, and metabolic defects). The categories of cancer that are influenced by extrinsic factors including many tumors of the skin and mouth, the respiratory, gastro- i ntestinal, and urinary tracts, hormone-dependent organs (such as the breast, thyroid, and uterus), haematopoietic and lymphopoietic systems, all of which, collectively, account for more than three-quarters of human cancers. It would seem, therefore, that the majority of human cancer is potentially preventable.

Results of the Reactor Safety Study Summary of Results

The results of the RSS included estimates of the probability distributions for a variety of forms of harm early fatalities, early illness, latent cancer fatalities, thyroid nodules, genetic effects, property damage, and magnitude of the area in which relocation and decontamination would be required. These results were presented in the form of graphs of the probability of occurrence as a function of the magnitude of the harm. Thus, for instance, the calculated probability of an event that would cause more than 1 latent cancer death per year was about 3 x 10 5 reactor-year (RY) the probability dropped to 2 x 10 6 RY for more than 100 latent cancer deaths per year 12, p. 97 . It is, however, notable that no increased risk of leukemia, an entity known to appear within 2-3 years after exposure, has been identified more than 10 years after the accident. 28, p. 517

Disasters Nuclear Accidents

Radioactive emissions of particular concern include strontium-90 and cesium-137, both having thirty-year-plus half-lives, and iodine-131, having a short half-life of eight days but known to cause thyroid cancer. In addition to being highly radioactive, cesium-137 is mistaken for potassium by living organisms. This means that it is passed on up the food chain and bioaccumulated by that process. Strontium-90 mimics the properties of calcium and is deposited in bones where it may either cause cancer or damage bone marrow cells. Fears of terrorist attacks on nuclear power plants have prompted state and local health offices to distribute supplies of potassium iodide pills, known as KI, to be taken in the event of a release of radioactive materials. KI blocks the intake of radioactive iodine by the thyroid and helps prevent thyroid cancer. The pills were provided by the Nuclear Regulatory Commission.

The Chernobyl Disaster

The health impacts of the Chernobyl explosion will never be fully known. It is estimated that some three million people still live in contaminated areas and almost ten thousand people still live in Chernobyl itself. The plant itself was not fully shut down until nearly fifteen years after the disaster. Studies by the Belarus Ministry of Health, located approximately eighty miles south of Chernobyl, found that rates of thyroid cancer began to soar in contaminated regions in 1990, four years after the radiation release. Gomel, Belarus, the most highly contaminated region studied, reported thirty-eight cases in 1991. Gomel normally recorded only one to two cases per year. Health officials in Turkey, 930 miles to the south, reported that leukemia rates are twelve times higher than before the Chenobyl accident.

That does not mean, however, that indoor radon is 1000 times more harmful. A collective dose calculation is meaningful only if the LNT theory is valid. Considering only directly established harm to human health, Chernobyl caused 31 deaths among workers and has led to an increase in thyroid cancer in children (see Section 15.3.4), whereas there is no specifically identifiable harm from indoor radon.

Some countries such as the USA are resisting moves to renew their aging power stations while others, such as France, are enthusiastically growing their nuclear real estate. The link to bombs, the dangers of radioactivity, waste disposal and safety, all feature in the debate, which is 'spun' beyond rationality by the degree of polarisation demonstrated by the protagonists.
